Instructions
1. Click on the link below to fill out the team/player registration form. To protect player’s privacy, only list MONTH/YEAR for date of birth.

Download, Save, and Fill out the Form. When completed, email to us.
2. Come back here to submit your team entry fee of $550, (deadline for entry fee is Sept. 1) or if you are entering as an individual player $60, through Paypal (send to [email protected]). Fees go up after Sept. 1, if there are spots available.
3. Individual players should just pay the $60 fee and we will contact you if there are vacancies and we can accommodate you.
4. If we have questions about your registration, we will contact you.
5. Rosters and entry fee must be accompanied together or your spot will not be guaranteed. This rule will be enforced this year.
About two weeks prior to the tournament, we will email the manager the team’s roster so they can have the Release of Liability document signed by all players prior to their first game.
